I am using this with Cox internet. I had to go into the store and have them disable the old modem and add this as the new modem on the account.
Once it was registered, it took a few reboots and about 20 minutes but I got it working at home just fine.

I had to call Netgear support. On the firmware that shipped with the router there is a problem with the 2.4 or 5 ghz band dropping. There is a list of tweaks they will make you do to make the connection more stable, which basically slows down the band.
I am not entirely sure why this is an acceptable fix for an expensive product, but I am going to give it a shot anyway. I may return if it drops again.
For the price I expect this thing to work out of the box with minimal config.

Why is there a 12 month for $49.99 and this one is $59.99 and both seem to be able to be used on PS3, PS4, PSvita?
PSN price increased from 49.99 to 59.99 in September of 2016. If you see a 49.99 price, it is because someone is using old signage. Do the store a solid and let them know so they can fix the sign. If it is listed on the site its because best buy didnt remove the old pricing.

There are two cards at Best Buy for the 12 month PlayStation plus...one for $49 and the other for $59.... are they the same?

I dont believe your cable box has the ability to do this. No standard cable box does.
I also dont know of any smart TV's that allow for a USB device like this to be plugged into a port and used for that purpose.

Can you record a TV show
In response to your query - you can't directly record a TV show onto a DVD. You would need to convert the TV signal to a digital signal, and then record that signal to a format that you would then burn to the DVD.

This router should be fine for wireless devices and Netflix. If you want consistent HD streaming with netflix, I would always suggest a hard wired connection to the device.

You can backup your files to it, but it is kind of small by todays standards. This would be better for something temporary, and you should probably back up your files in the cloud.

Is it bootable?
You can boot to this, if you format it first. No USB drive is bootable without configuring it first. Its just a blank drive.
