CD/DVD sleeves are pretty standard. These are good quality and serve their purpose well. They are a bit higher priced than some but, since not everyone stocks them, I'm just glad to find them.

How many dvd's does this hold?

They're made for a single disk, but two could be slipped in....or even three or four (though that's pushing it).

Are the sleeves paper or plastic? If paper, do you have plastic in a similar form?

They're paper, with a clear overlay so you can see the disk inside.
