Trackball is the only way to go. Moving around the screen is easy on the wrist and accurate.
I would recommend this to a friend!

Does the scroll wheel tilt sideways for horizontal scrolling?

The rollerball sits at an angle, therefore in perfect position for scrolling any direction. I wouldn't use any other design.
