Well, when it works it sounds fantastic with the rear speakers. Unfortunately certain commercials or sources cause the bar to buzz and I don't know how to stop it. I've tried HDMI ARC and optical, they both do it. Plus, when it's on ARC it changes to the other HDMI input instead of arc which means I get no sound. Very frustrating.
No, I would not recommend this to a friend.

I have purchased many of these, both for myself and for others and will continue to recommend them to anyone that needs a programmable remote. Internet is not required to use these, only for setup and if you want to use the app.

Will this connect via an optical cable or do you have to use hdmi only?

It supports optical also, yes.

Does this unit work with Comcast Xfinity desktop boxes

Comcast says it DOES NOT, curious if anyone tried it.
This will work with all XFINITY cable boxes. X1, legacy, DTA... everything

Does this remote blast ir out as well as the smart hub? my tv is in another room with no wiring for the ir expansion in place?

The remote does nothing. The hub is what emits the IR signal, but also includes an IR blaster with support for a second one. You can use a 2.5mm extension cable if you need more length to the other room. Not sure what the limit is.

Can this device replace a TWC remote?

If the box uses an IR remote originally, yes.
