With the Motorola SB6121 Modem, does it offer a wireless router, or is it one that has to stay plugged into a computer?

no this one don't have built in router. will work with router and don't have to stay plugged in to computer.

Does this have a Cable type plugin in the rear of the modem?

Like my current ISP provided one does?
Yes it has a cable plug in. Work great for cable.
