Good wall mount, but watch out while adjusting

Price was very good compared to the alternatives. Pretty easy to follow instructions and comes with a lot of extra bolts to adapt to your particular application. Comes with zip ties and short plastic conduit to help manage cables. I mounted it with a 40" Visio TV. Be careful when adjusting: there are set screws to loosen for each direction of travel and they are not readily visible - read the directions carefully to see where you loosen BOTH ends of the bracket to swivel (they are hidden!). I only loosened one end and ended up inadvertently forcing the other end.
Now the only thing I didn't like: the two brackets that mount to the TV were too close together so that when you mount the swivel bracket between them you have to push with more force than is safe for a flat screen. I actually had to use a hammer to get enough force to tap it in. If I had the right tools (a round rasp file), I would have just widened the slots that the screws mount through into the back of the TV; the slots only need to be a 1/16th of an inch wider and I would have given this bracket 5 stars. This is the only flaw I saw in the wall mount's design. If you have a different TV this may not be a problem for you.

How do you remove mount from TV for repair of TV?
Pretty simple - there are four Phillips screws going through the bracket into the back of the TV. Just swivel the tv out from the wall and have someone hold the TV while you unscrew the four screws.

Does the 40" max size refer to measuring the screen diagonally or horizontally?
I have an LG 42LV5500. The screen is 40" measured horizontally but it is considered a 42" TV because they measure the screen diagonally. So is this model LG compatible with this wall mount? The weight of the TV is 29 lbs.
The measurement is diagonally. It would void the warranty and you would assume all risk, but I'm pretty certain that the TV weight would not be an issue at all - my 40" visio is the same weight on my bathroom scale. What may be an issue however, is the mounting screws on the back of the TV - there are four of them, two on each side. The two on each side are four inches apart vertically and the two pairs are 4.25 inches apart horizontally. If this matches up with the holes on the back of your TV (i.e. the holes are a rectangle that is 4 by 4.25) you should be ok.

When my tv is not extended and is fully up against the wall it is not centered to where I want it to be. Is there anyway to fix this?
I've already moved the tv along the holes in the brackets. The tv is also bracketed into the stud closest to the center.
I had the same problem, only way I could solve it was to center the TV where I wanted it when it's up against the wall and then marked where the holes into the wall would be. Of course this no longer lined up with the studs and was just in the dry wall. Home Depot sells dry wall mounting studs that are rated to hold up to 100 pounds in normal dry wall (was less than $5 for a package of two). Used these drywall anchors and it worked perfectly. The down side is you must be VERY careful to NOT put extra force on the wall anchors when adjusting the swivel now (you don't realize how much force you are adding to the weight of the TV when you start pushing or pulling on the TV to move it around). You must loosen up all the set screws while someone supports the TV and then move it to where you want it and retighten. You also assume all risk if it does pull out of the dry wall since the instructions tell you very clearly you MUST mount into a stud. Mine's been mounted into the drywall alone for two months now with no issues.

Can I turn the mount 90* so that the TV is in Portrait view instead of landscape view? And if so, can it be done easily on the fly, or does it require using tools to tighten/loosen parts.
The mount doesn't adjust that way (i.e. does not rotate clockwise and has no ability to). If you are talking about attaching the brackets to the back of the TV so that they are screwed in horizontally instead of vertically, that looks like it will work. There are 4 screws holding two brackets to the back of the TV (two on each bracket). They are 4" apart vertically and about 4.25" apart horizontally - looking at the extra mounting holes to accommodate different TVs, it appears that it will also accommodate mounting the brackets to the back of the TV in this way. This would not be easily modifiable on the fly - would have to have someone hold the TV, while a second person uses a Phillips screwdriver to remove the four screws into the back of the TV.

What is distance between wall bracket mounting holes?
Looking for distance between holes on the bracket that mounts to the wall?
The holes where the bolts go through the bracket into the wall are 9.5 inches apart
