Does this get very hot anywhere on the outside? I have breathing issues & can no longer use my regular oven as a result. Does this give off any smoke, etc or strong smell other than aroma of whatever you're cooking/baking? Thanks

When the oven is turned on for the first time, it may emit vapors. This is due to the protective substances on the heating elements. It is safe and not detrimental to the performance of the oven. But these odors won't be present after the first time.

Yes, the outside of the oven does get hot.

Should I hear any airflow during convection but I do during superconvection. Is this correct?

Convection can be quite quiet. You will definitely hear more during super convection (air fry).

can this be used on a shelf under the counter? how much room would it require all around for safety etc. There would be a back on the shelf.

The Combi Wave™ 3 in 1 requires a minimum clearance 8 inches /20cm away from side walls and 8 inches from top. It cannot be built into a cabinet that has less clearance that this.
