I was shopping for a shelf to house my husband's electronic items. I had an idea of the look I wanted to achieve, but did not specifically know what I wanted, the sales person suggested this item and requested that I try it. I did and it couldn't be better it is just "On the Money" a great asset to my husband's "Man Cave" instead of multiple shelves now I have all his electronic gadgets on one shelf.
I would recommend this to a friend!

does it have to be wall mounted or can it be mounted to the tv holder

I don't think it is possible to mount this item on a TV monitor, it need to be free-standing and anchored to a solid surface.

Can I mount this in a corner?

It fits best if mounted on a flat wall and not on an angle. Since I am not mechanically inclined a friend mounted it for me, it took him all of maybe 10 minutes if that long.

Can I add another shelf to this or do I have to buy another model .

My understanding is you'll need to purchase another shelf. Each set is specific to that shelf.
