Does the scroll wheel tilt sideways for horizontal scrolling?

It does not tilt, but you can press it like a button for horizontal scrolling.
When you press it, you will see something that looks like the 4 points of a compass. You move your pointer any direction from there and the screen will scroll that direction (including diagonally). The father the pointer is from the compass, the faster the scrolling.

What is ball diameter ?

The opening the ball rests in is 1 3/8 inches wide, so the diameter would be close to that.

The bottom one is the "back" button (such as on a webpage) and the top one is the "forward" button.

You say a dongle is used in the USB port to use the wireless mouse but suppose I need to use a flash disk. Is there more than 1 USB port so I can use the wireless mouse and flash disk at the same time?

That is a question of your computer or laptop. Using the mouse requires one USB port to be used at all times. If your computer only has one USB port, then anytime you use the flash disk you would be disabling the mouse. Most laptops have at least 2, most desktops have at least 6 USB ports.

I see vertical scrolling with the middle button, but does this trackball mouse offer horizontal scrolling?

Yes.
The scrolling wheel can be pressed like a bottom. On your screen you see the 4 compass points and your pointer. Move the ball slightly the way you want to scroll and the screen will scroll. The father you move the pointer from the compass, the faster scrolling will happen. You are able to scroll any direction (diagonally included) in this mode.

This version only has the tracker ball on the left side (used for the thumb on the right hand), my left handed family members use the mouse with their right hands, so have used this mouse as well as my right handed family members.

It requires a receiver that plugs into a USB port (not USB-C)

Does the USB receiver include with trackball wireless?

Yes, it comes with the wireless receiver that plugs into a USB port. I have had 2 on different computers, they don't interfere with each other.

Not directly.
The mouse uses a USB wireless receiver (it comes with the mouse) and I pads don't have USB ports. I don't know if newer ones with a USB-C and an adapter would work or not.
