With a 65" TV that is 33" tall attached to the mount, how tall is the entire assembly?

I need to put it inside a cabinet that is just over 62" high. Am I correct in assuming that the TV will make it taller than the 55" cited in specs?

It will depend on the height you use for the mounting frame and where the mounting holes are located on the back of your television. Yes, the height will be taller than 55.25"; this is the overall height to the tip of the monitor brackets on the highest position. Is the middle shelf adjustable for height?

I need to fit a center channel speaker on the middle shelf and it's nearly 12" tall. Is the middle shelf adjustable for height or if need be can I leave it off the stand?

Yes, the middle shelf is adjustable; on the lowest position the height is 9.1" H. We wouldn't recommend removing it completely from the stand.
